Epidemiologische Daten

Mit Hilfe der gesammelten Daten gelingt es uns epidemiologische Kennzahlen zu errechnen. Dabei richten wir unser Augenmerk auf die Bestimmung der Inzidenzen und Prävalenzen sowohl des Myokardinfarkts als auch der Risikofaktoren.

Einfluss der sozialen Lage auf die Gesundheit

Bestimmte Regionen in Brandenburg mit schwierigen sozioökonomischen Situationen weisen eine höhere Prävalenz an Myokardinfarkten bei über 80-jährigen auf. Es scheint denkbar, dass ungünstige soziale Faktoren wie Bildung und Einkommen einen Einfluss auf die Gesundheit haben. Zusätzlich prüfen wir, ob die o.g. sozialen Aspekte einen Einfluss auf die Inanspruchnahme ambulanter kardiologischer Diagnostik und damit die Früherkennung der KHK haben. Wir stellen uns also die Frage, ob soziale Deprivation ein eigenständiger Risikofaktor für kardiovaskuläre Erkrankungen sein kann und sehen diese neben der Epidemiologie als einen Schwerpunkt unserer Registerarbeit.

Lebensqualität bei Ischämischer Kardiomyopathie

Der akute Myokardinfarkt ist für Patienten ein einschneidendes Ereignis. Oft sind betroffenen Patienten erstmalig mit einer stationären Behandlung, lebenslanger medikamentöser Therapie und ambulanter Kontrolle konfrontiert. Ein Vergleich zwischen Patienten mit stabiler KHK und Patienten mit Myokardinfarkt im Hinblick auf die Lebensqualität lohnt sich daher zu untersuchen.

Regionale medizinische Versorgung

Wir möchten für unser Bundesland die Überlebenschancen bei Eintreten eines Herzinfarkts verbessern. Daher müssen vorher die Gründe gefunden werden, warum mehr Menschen an dieser Erkrankung versterben als anderswo. Die regionalen Gesundheitsunterschiede weisen ein ausgeprägtes Ost-West-Gefälle auf. Um diese Frage zu klären, sind neben stationären Daten auch Informationen über die ambulante und notfallmedizinische Versorgung zu dokumentieren. Langfristiges Ziel ist die Senkung der Prävalenzen von kardiovaskulären Risikofaktoren und deren Folgeerkrankungen sowie eine positive Beeinflussung tödlicher Erkrankungsverläufe, im Besonderen beim akuten Herzinfarkt.

Echokardiographische Parameter zur Prognosebestimmung

Nach Myokardinfarkt erfolgt noch während des stationären Aufenthaltes die Durchführung einer transthorakalen Echokardiographie. Echokardiographisch bestimmbare Parameter wie z.B. die systolische und diastolische Pumpfunktion können nach Myokardinfarkt reduziert sein. Daraus ergibt sich die Fragestellung, ob echokardiographisch messbare Parameter einen Einfluss auf den Verlauf nach Myokardinfarkt und die Hospitalisierungsrate haben.

Dysfunktionales HDL als Frühmarker der Artherosklerose

Das dysfunktionale HDL (oxidiertes HDL) dient als Frühmarker der Atherosklerose und korreliert im Vergleich zum Gesamt-HDL besser mit der kardiovaskuläre Mortalität und Morbidität. Im Rahmen einer vom Herzinfarktregister getrennten prospektiven Studie werden Patienten mit subklinischer Atherosklerose, die aus studienunabhängigen Gründen stationär aufgenommen wurden, rekrutiert. Die studienspezifischen Maßnahmen beinhalten laborchemische Parameter, die Messung der Intima-Media-Dicke, des Ankle-Brachial-Index sowie telefonische Nachverfolgung.
